Module Description: The aim of the module will be to empower Mental health Nurses work at advanced level in leading and developing integrated person-centred care within the community mental health services in order to deliver better outcomes for
people, professionals and organisations. This module will situate advanced practice in an integrated care context reflecting contemporary national (HSE 2015; DoH 2016,
2017; Government of Ireland 2019) and international (WHO 2005; 2007; 2010; 2017) health and social care policy. An integrated care approach is recognised by the WHO as
fundamental to addressing Goal 3 of the United Nations Sustainable Development Goals (UN 2015). Integrated care recognises the changing nature of
population needs. This seeks to move away from managing acute, episodic care to a more holistic, person-centred, longitudinal approach to care and case management. This
in turn requires Mental Health Nurses to re-examine their philosophy and practice. In doing so practitioners will reconsider the nature and focus of their work within and
outside traditional institutional relationships. To prepare registered Psychiatric Nurses to screen and assess those with mental health issues and secondary Physical Health issues and to improve detection and management of mental health issues, including risk of suicide, in community settings.
 
Module Learning Outcome
On successful completion of this module the learner will be able to:

MLO1 Critically examine the core tenets of integrated care to ensure ethically responsive practice in dynamic community environments
MLO2 Critically apply relevant theory of health promotion priorities including the social determinants of health in the area of Community Mental health Nursing and Implement health promotion strategies for service users in accordance with public health agenda to develop person-centered, evidence-based, integrated care processes
MLO3 Critically apply relevant theory of holistic consultation and assessment skills to provide evidence-based care for service users attending with a diagnosed or undiagnosed mental health issue and secondary physical health issues.
MLO4 Critically employ the ability to perform a Mental health Nursing assessment, plans and initiates care and treatment modalities within agreed interdisciplinary protocols to achieve service outcomes and evaluates their effectiveness.
MLO5 Critically apply relevant theory to service planning and budgetary processes through use of audit data, specialist knowledge and the use of digital health programmes.
MLO6 Apply competencies in planning, designing, implementing and evaluating person-centred integrated care

The student will undertake a case study that
draws upon indicative content/reflects learning
outcomes. In the assignment students will
critique an aspect of service provision drawing
on the principles of the Mental health Commission National Quality framework and
make a proposal for change based on these
